I disagree with the precision thing of the good play through. Yes you have to worry about protecting bystanders and that is necessary as you said Kuo drives that in the narrative so why outside of the narrative would cole disregard that outside of it? But with that being said the little affects that you get for hurting a bystander is little to none. I haven't got to the second city but i have max good karma and usually get hit with injuring or killing a bystander. but the good things you do in the mission will always overpower that.

But at that point, why is it included? There should be a threshold. If the game sees that you're killing a ton of civilians then it should then count that as an evil deep; however, if the user has also killed enemies and one or two civilians get hurt, the game should be intelligent enough to compensate for error.
