Update: To enable gore, do this

When starting the game add +set lock_blood 0
When you start the game, pull down the console with shift-~
Type 'menu lock'
Turn on all of the gore and set a new password (blank works).
Then restart the game. The menu will appear and the game won't have gore locked out anymore.
